Reporting Safety or Quality Issues, Comments, and Concerns

Any patient of CIBD has the right to express to any staff member, the Patient Advocate or a state agency, at any time, and without retaliation, a complaint or a grievance concerning care and treatment. Concerns about patient service should first be directed to CIBD’s Compliance Hotline at: 657-375-0499

If concerns cannot be resolved, a patient, family member, community representative or employee of CIBD has the right to notify Joint Commission regarding the quality of care provided, safety of care provided, or safety of the environment in which care is provided. Anyone believing that he or she has pertinent and valid information about such matters may notify Joint Commission via any of the following methods:
