Foundation Registration Application

The Conference Planning Committee, led by the Hemophilia Treatment Center at Kapi’olani Medical Center for Women and Children, invite two leaders from each local Hemophilia Foundation Chapter to apply to attend the 2025 Western States/Region IX Hemophilia Treatment Centers’ (HTC) Annual Conference.

An application is required to attend. Please submit a completed application form by January 31, 2025. Separate forms must be filled out for each attendee. Once the application is accepted, you will receive a formal registration confirmation.
