Last Modified: January 8, 2018

This Privacy Policy applies to cibd-ca.org (the “Website”) owned and operated by the Center For Inherited Blood Disorders (“CIBD”). This Privacy Policy describes how CIBD collects and uses the information we obtain through the Website.

CIBD Collects and Stores Information You Provide Us

CIBD (also referred to as “we” or “us” in this Privacy Policy) collects the information voluntarily provided to CIBD by visitors to the Website (also referred to as “Visitors” or “you” in this Privacy Policy) when you make an appointment, fill out forms on our Website, or otherwise contact us via the Website or email links contained in our Website. The information that we collect on or through our Website may include your first and last name, email address, telephone number, organizational affiliation, and any other information you provide when you use the CIBD Website or otherwise contact us, such as in connection with requests for an appointment or information, or access to our informational videos.

We do not collect any information from you using automated means such as cookies, beacons, or online data analytics tools, so you will always be aware of the information you have given us.

CIBD Does Not Track You Online

CIBD does not track you when you visit other websites or engage in other activities online, so we do not register “do not track” signals from your browser.

How Is Visitors’ Personally Identifiable Information Used, and With Whom Is It Shared?

CIBD, together with its clinical and community partners and other authorized third parties, uses Visitors’ personally identifiable information to provide you with information, education, support, and services, and to otherwise communicate with you. We will use your telephone number and email address to contact you and send you messages, including to respond to appointment requests. We may use your mobile telephone number to call you or send you text messages, if you have agreed to receive them.

Information collected from Visitors is shared with CIBD, its clinical and community partners, and other authorized third parties. We also reserve the right to share or transfer your information in connection with a sale or other acquisition of CIBD, provided the acquirer agrees to honor this Privacy Policy.

We do not share your information for third parties to market to you, or for any other purpose that is not related to information, education, support, and services provided by CIBD.

Only CIBD employees and authorized representatives, who have a “need to know” may access Visitor information stored by CIBD, and everyone who is allowed to access Visitors’ information has agreed to keep it confidential.

CIBD cannot guarantee that your information will not be compromised, and CIBD expressly disclaims any liability for the loss or theft of information you provide us.

Aggregated and Non-Personally Identifiable Information

CIBD may use information submitted through the Website on an aggregated, non-personally identifiable basis, to improve our Website, learn about our Visitor community, and determine whether information, education, support, and services provided by CIBD are reaching our intended audience. Such aggregated, non-personally identifiable information may also be shared with third parties with whom CIBD partners to provide its information, education, support, and services.

Links and Third Party Websites

The CIBD Website may contain links to other websites. Please be aware that we are not responsible for the privacy practices or the content of such other websites. We encourage our Visitors to read the privacy statements of each and every website they visit. This Privacy Policy applies solely to information collected by us through the CIBD Website and does not apply to these third-party websites. The ability to access information of third parties from the CIBD Website, or links to other websites or locations, is for your convenience and does not signify our endorsement of such third parties, their products, their services, other websites, locations, or their content.

Children Should Not Provide Information to CIBD

CIBD’s Website is not intended for use by children under the age of 18, and CIBD requests that children DO NOT provide information to us. If CIBD becomes aware that information has been submitted by a child, the information will be deleted from the CIBD database.
